A government investigative team Tuesday issued a report soft-spokenly but formally blaming avoidable mistakes by contractors for the failures Nov. 15, the fifth day of the fifth mission of space shuttle Col umbia. The question of money penalties was still pending for the contractors — United Tech nologies Hamilton Standard Di vision of Connecticut and its subcontractors, Carleton Con trols Corp. of New York and F.W.
