Canada failed to send report
on plane crash, officials say
WASHINGTON (AP) -
Canadian officials said Monday
they never sent their U.S. coun
terparts a preliminary report into
a 1989 crash under conditions
similar to those of the recent US-
Air crash at New York's La
Guardia Airport.
Last week, the head of the
Canadian inquiry implied that
the March 22 crash at La Guardia
might have been avoided had
U.S. officials heeded his panel's
report, which U.S. officials said
they never received.
“As reported in our telephone
conversation of this morning, no
copies of our interim report of
December 1990 were forwarded
by the commission to any agency
of the United States govern
ment/' according to a memo ob
tained by The Associated Press.
It was signed by Gordon A.
Haugh, a spokesman for the
Canadian inquiry panel, and ad
dressed to Frank McCabe, special
assistant to the director of the
Federal Aviation Administration.
“I realize the embarrassment
that my remarks could cause and
apologize for this unintentional
misinformation/' Haugh wrote.
Steady snow was falling
when USAir Flight 405 crashed
into Flushing Bay upon takeoff,
killing 27 people and injuring 24.
The plane, a Dutch-made
Fokker 28, had been de-iced
twice before takeoff with an alco
hol-based solution called Type 1.
The second de-icing occurred 30
minutes before takeoff.
